Active sessions list and revoke actions now cover every session

The Active sessions list previously only requested WorkOS's default page of sessions: up to 10. An account with more than 10 active sessions saw an incomplete list, and, more importantly, Sign out of all other devices and the automatic revoke that happens when you change your password only reached sessions on that first page; any session beyond it stayed signed in.

This is now fixed: the list requests every active session (up to 100), so both bulk actions revoke all of them, not just the first 10.

While fixing this, the list also got two smaller improvements: each row now shows a human-readable device name (for example "Chrome on macOS") instead of the raw browser string, and the list is a paginated table: five sessions per page, with Previous/Next controls once you have more than one page.
