Fixed a duplicate row for your first custom evaluation

On the evaluation launch screen, creating an agent's very first custom evaluation added it to the "Custom" checklist twice instead of once. Reloading the page cleared it up, and every custom evaluation created afterward was unaffected. But that first one showing double made the checklist look broken.

Your first custom evaluation now appears once, immediately. No reload needed.
